Abstract

Implementing a web-based information system is crucial for hospitals to improve the quality of healthcare services and administrative efficiency. Currently, the General Consent form filling procedure at Hospital X is still performed manually using paper forms, which leads to a high risk of incomplete data, damaged or misplaced files, and hinders the legal and claims verification process. This research aims to design an information system to detect and address incomplete general consent forms internally. The Waterfall method used includes requirements analysis, system design using UML (Use Case, Activity, and Class Diagrams), implementation with Sublime Text and MySQL, and functional testing. The results show that the developed system successfully validated key modules such as inpatient registration and digital general consent letter creation. This system is expected to improve the time efficiency of medical personnel and ensure the accuracy of medical record documents, providing legal protection for both patients and the hospital.
